AI copilot drafted my safety report, almost got us fined $12k in Portland

I manage a small construction crew and last Thursday our site inspector flagged a report I filed. The AI writing tool I use for summaries generated this perfect sounding text about our crane inspection. Problem is it invented a certification number that doesn't exist and cited a date 3 days in the future. The inspector caught it because he knew our equipment. I had to choose between letting the AI handle all the boring paperwork or going back to writing everything by hand. I picked a middle route, AI drafts but I verify every single number against our log book now. It takes an extra 20 minutes per report but beats an OSHA headache. Has anyone else had an AI assistant fabricate details in official documents without warning? How do you catch it before it goes out?

Treat it like a second set of eyes that lies sometimes. I run every generated number through our actual log before it goes anywhere, and I flag anything that looks too clean. That extra 20 minutes is cheap insurance against a stop-work order.
