My domino joiner slipped and ruined a set of cherry cabinet doors today

I was working on a set of custom kitchen cabinets in my shop outside Columbus, Ohio. Had 14 shaker doors all lined up, and my Festool domino joiner slipped on the third one because I didn't check the fence tightness. Gouged a nice chunk out of the center stile on a customer's piece I already had sanded and stained. I ended up having to cut a whole new stile, re-sand and re-stain it, and that set me back about 3 hours for just that one door. Has anyone else had a similar slip with a plunge router or joiner, and what's your go-to trick for keeping the fence locked down tight?

Oh man, I've been there with a router bit grabbing and chewing up a nice piece.

Oh great, so it's not just me then. My router decided to take a scenic detour through a piece of walnut last month and I swear it left a groove that looked like a small trench. My go-to trick? I just give the fence a good whack with a rubber mallet after tightening it now. Seems to do the trick about 80% of the time. The other 20% I just accept my fate and get good at making sawdust.
