Double exposures made my whole roll unusable and I didn't notice for 3 months

I kept shooting a roll of Portra 400 in my Pentax K1000 and every frame came back with ghost images of my kitchen window, but I thought it was a light leak or something. Turns out the film advance lever was slipping after about 20 shots, so it was re-exposing the same frames without me realizing. Has anyone else had a camera silently do this to them, and how long did it take before you figured out the real cause?
